In order to complete the first step towards the registration process, you will need to fill out a form after reviewing the "Programs Overview" page.
This form is called the "New Client Registration Intake Form."
It is a 15-20 minute questionaire providing us with extra information about what your needs are and why you would like to join as a member.
Some required fields ask for additional information which may or may not be relevant to your application. While we appreciate as much information you're comfortable with, you can follow the instructions on how to properly decline answering some supplmental questions.
Please note, depending on the type of membership requested, it will help us out by completing all fields at a moderate length. You may also use the "Make A Payment" button to pay your Application Fee.
